First, you and the instructor realize that this product has been discontinued? I simplely would just author this using web technologies. You are going to encounter too many issues (state number limits for example) will trying to buid this. In addition, there is no one left using this application to assist to with issues. I have finally uninstalled from my machine, so I could not even test anything if asked.
